Attention Quest Lab Users
[9/24/2012] - It has come to the Welfare Fund's attention that random Quest Labs are asking for a $40 "courtesy" co-payment from all patients regardless of the coverage they have. Empire BlueCross cannot contractually prevent Quest Labs from asking for this "courtesy" co-payment. Our EPO and MediBlue co-payment is $0 (zero) for using Quest Labs. Therefore, do not pay any amounts to Quest Labs. Quest Labs continues to be in our network and you have full coverage. Should you have any questions about this matter please contact the Fund Office at 212.465.8888, Option 4. more

2011 income allocation and administrative charge for the Security Benefit Fund
[6/7/2012] - The 2011 income allocation and administrative charge for the Security Benefit Fund have been posted to participants' accounts. For your information, the income allocation percentage was 5.0497% of the participant's average balance in 2011.  This average balance is determined by taking the four quarterly closing balances in 2011, adding them up and dividing by four.  This average balance is then multiplied by the percentage and the amount is debited to the participants' accounts. The administrative charge is $297.33 per participant and every account that can sustain the charge has had this amount deducted.  If an account had less than that amount, the charge was whatever amount the account could sustain.  The following members will not be charged at all; pensioners with a balance of $2,500 or less and deceased members with a balance greater than $10.00. If you need additional information, please call the Security Benefit Fund dept. at 212 465-8888 option 4. more
